The Section office does not accept eligibility forms by fax. They must mailed.

Eligibility forms continue to arrive in the Section office with missing or incomplete information (areas unsigned, unmarked boxes, etc.). Incomplete eligibility paperwork is mailed back to the Athletic Director of the new school with notification of the areas that need attention.
Please use due diligence and review all information that is submitted to the Section office for completeness to avoid a delay.

When submitting eligibility paperwork, please make sure that:
• areas with fill-in lines are not left blank
• all appropriate boxes have been checked
• both schools (former and new) have signed the appropriate areas
• any attachments necessary for the Section to rule on the matter are included
